Required Information

All claims submitted via the claiming app, online or via post must have:

  • A legible receipt that has been supplied by the provider that clearly outlines:
  • Date of Service
  • Item numbers of services performed
  • Provider Number
  • Cost of services provided – If multiple services have been provided each service must have an item number and the cost per service itemised.

For claims that have an MBS (Medicare Benefits Schedule) number you must supply one of the following pieces of information as evidence:

  • Medicare statement of claims and benefit
  • Medicare claims history
