Get a Replacement Card

If you have lost or damaged your National ADF Family Health Program card, ordering a new card is quick and easy.

You can call us on 1300 561 454 or simply log on to your Online Member Services account, and select the ‘Useful Tool’s tab. You will find the option to order a new card here. Simply follow the prompts and a new card will be sent out to you within two business days.

 

Additional Card

If you wish to order an additional card, please call us on 1300 561 454. Please don’t order it through Online Member Services as it  will cancel your old card.
